Ears are Hard!  There is no two ways about it, fashioning a realistic looking ear on a cloth doll is a tedious task, and never quite fully accomplished to my satisfaction.
Be sure to remeber the quadrants of the face when placing the ears on a doll.  When I do make ears, I usually sew them into the side seams of the head and then tack them back.
As you can see, a realistic ear is a bugger to sculpt, realistically, even draw realistically.  The ear is designed to capture and transfer what we hear in the most efficient way.  Its design mirrors it's complicated function.
You can create the illusion of an ear by using the diagrams above as pattern, you will have to make it proportionate to your doll's head.  First sew the ear RST.  Leave the end of the tab open to turn the ear to the right side.  Top stitch the ear as shown in figure 2.   Good luck.
